If you are or were a patient of Columbus Regional residing in Indiana who logged into a patient portal account between November 1, 2017, and June 30, 2022. you may be a Class Member and eligible for Settlement Benefits. Excluded from the Settlement Class are: (1) the Judge in this case, and the Judge’s family and staff; (2) Columbus Regional and its officers, directors, and related companies; (3) anyone who validly excludes themselves from the Settlement; and (4) attorneys for the parties.
If you are a Class Member and wish to receive Settlement Benefits then you must submit a Claim Form by September 19, 2026.
Columbus Regional has agreed to pay for two benefits that are available to all Class Members.
CyEx Privacy Shield Pro. All Class Members are eligible to enroll in one (1) year of CyEx Privacy Shield Pro,and were mailed their enrollment codes. If you no longer have your enrollment code, please contact the Settlement Administrator. This comprehensive service is designed to restore users' privacy and anonymity, and includes:

Cash Payment. All Class Members may also claim a $25.50 cash payment. You do not have to provide any proof or explanation to claim this payment.
To start your online claim form, you must login with your Unique ID and PIN. You can find your ID and PIN on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailing info@ColumbusRegionalSettlement.com. Please request your ID & PIN by providing your full name and mailing address.

The deadline to submit a Claim Form online or by email is September 19, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than September 19, 2026.
